Whether you are new to treatment or transferring care from another practice, I will need to get some information from you to understand what kind of treatment you are seeking.

If you would prefer to be contacted on a weekend or outside of normal business hours, just let me know. I can usually schedule your first visit within 24-48 hours of your request.
Payment Policies
Aspire PDX does not bill insurance, and I am not contracted (in-network) with private insurance carriers, Medicare, Medicaid, or the Oregon Health Plan (OHP). Payment is due at the time of service. Accepted forms of payment include credit/debit cards, or personal checks. At each office visit, a superbill will be provided summarizing the services you received, so you can submit a request for reimbursement (if eligible) from your health insurance or flexible spending account.
If you have health insurance, it is very important that you understand your medical and drug coverage benefits before scheduling an intake appointment at Aspire PDX. If you are insured by Medicare or the Oregon Health Plan, please be aware that you will not be eligible for reimbursement for services provided by this office.
The fee for services provided at Aspire PDX does not include medications or laboratory charges.
Fee for Services
Intake includes initial consultation and all office visits needed to stabilize your care for the first 21 days of treatment: $400
Monthly follow-up visits (25-30 minutes): $180
Weekly follow-up visits (15-20 minutes): $100
